The parents of a 28-year-old cargo runner crushed to death while working at San Francisco International Airport in January filed a wrongful death lawsuit Friday, arguing that his death was the “foreseeable consequence of overlapping failures.” 
The parents of John Brandon Lacayanga sued the city and county of San Francisco, Ground Services International and Dubai National Air Travel Agency, which provides aircraft ground handling, cargo, travel and flight catering services. 
Lacayanga was found dead under a cargo pallet he had been towing, the Chronicle previously reported. 
His cause of death was listed as “traumatic asphyxia,” according to the San Mateo County Coroner’s Office. 
Lacayanga was a resident of San Francisco. 
In a 28-page legal filing, Lacayanga’s parents said the death was “not a freak accident,” but resulted from his employer’s decisions to assign him inadequate and unsafe equipment, the failure of the city to properly maintain the airport roadways and poorly designed machinery that Lacayanga was using when he died. 
The lawsuit filed in San Mateo County Superior Court alleges negligence and seeks unspecified damages. 
Lacayanga was driving a tow tractor on one of the airport’s service roads when he hit a bump, fell out of his vehicle and was crushed beneath the cargo dolly that it had been pulling, according to the lawsuit. 
The lawsuit noted that Cal/OSHA cited Dubai National Air Travel Agency earlier this month for failing to ensure employees wore seat belts while operating a tug. 
The legal filing also accuses DNATA, Lacayanga’s employer, of past safety failures, including a similar accident at SFO seven years earlier in which one of its ramp agents “was ejected from a tow tractor after striking a curb,” resulting in serious injuries to the employee’s head, foot and arm, as well as a possible skull fracture. 
DNATA did not contest the violation, and paid a $7,335 civil penalty. 
The company has not yet responded to a request for comment about the lawsuit. 
The lawsuit also faults the city’s maintenance of the airport roads and “pavement irregularity,” which allegedly led to the bump that Lacayanga hit shortly before falling out of his vehicle. 
“Any loss of life is a tragedy, and our thoughts are with Mr. 
Lacayanga’s loved ones,” said Jen Kwart, spokesperson for the San Francisco City Attorney’s Office. 
“Once we are served, we will review the lawsuit and respond in court.” 
Finally, the lawsuit argues that Textron, the manufacturer of the vehicle he was driving, lacked proper safety features such as a roof or roll cage, or emergency braking.
